Abdominal ultrasound scan is the very first examination ordered for a patient with suspected liver disease. It shows the size of the liver, the regularity and sharpness of its contours, its structure, the vascular pattern and the condition of the intrahepatic bile ducts. Ultrasound reveals inflammation, liver cirrhosis and the presence of benign or malignant lesions. It does not, however, show the functional state of the liver.

Computed tomography (CT) is the next step in refining the diagnosis and provides detailed imaging of the organ when a liver lesion is suspected – a cyst, haemangioma, adenoma or adenocarcinoma. The scan is performed with intravenous contrast enhancement.

Needle biopsy of the liver is the international “gold standard” in diagnosing liver diseases. It is used fairly rarely thanks to the capabilities of imaging and laboratory diagnostics, but if the doctor has doubts about the diagnosis, or if an overlap syndrome (combined pathology) is involved – for example, primary biliary cirrhosis and autoimmune hepatitis at the same time – this method is indispensable. The procedure is carried out under ultrasound guidance and local anaesthesia: a puncture is made in the right intercostal space and a small piece of liver tissue is taken. The procedure is painless and safe.

Hepatology clinics and centres specialise in treating autoimmune hepatitis, drug-induced hepatitis, overlap syndrome, haemochromatosis, alcohol-related liver damage, non-alcoholic fatty liver disease and chronic viral hepatitis B, C, D and E. Particular attention  is paid to the treatment of hepatitis C and autoimmune hepatitis. The aim of treatment is to monitor the course of the disease over time and to minimise the unwanted effects of therapy.

Modern antiviral treatment protocols for hepatitis have significantly reduced the number of infectious complications and side effects of therapy.
