There are no impossible tasks! On 28 July 2020 the parents of seven-month-old Ksenia N. contacted us, having completely despaired of finding help in Moscow. The child had been given the diagnosis "post-resection short bowel syndrome following surgical treatment of a congenital malformation of the intestine, gastroschisis and jejunal atresia". The child had already undergone laparoscopic surgery in Moscow and a gastrostomy had been placed. The girl suffers from chronic enteritis and dystrophy, loses a great deal of fluid and electrolytes and is listless and exhausted. We were set the task of finding, within a very short time, a clinic and specialists who would agree to admit the girl for treatment. As usual in a situation requiring uniquely well-coordinated work by a multidisciplinary team of specialists — paediatric surgeons, geneticists, neonatologists — we approached one of the university hospitals of the state of Baden-Württemberg, a specialist centre for the diagnosis and treatment of
congenital and rare intestinal diseases and part of the ERNICA clinic network, an association of European clinical centres for the treatment of rare hereditary and congenital diseases and malformations of the digestive system and the gastrointestinal tract.
The same day we received a reply from the head of the paediatric surgery clinic saying that the hospital agreed to admit the child for surgery, which must be preceded by precise diagnostic work-up and by preparation for the operation, since in her current state of extreme dystrophy such an intervention cannot be carried out without risking complications. After the operation the child will be cared for by specialists in surgical rehabilitation. The chances of successful treatment are very high — 70-80%. We have passed all the information on to the parents and are awaiting their decision.
In mid-July 2020 a young woman from Moscow, Elena K., contacted us with a request for a "second expert opinion" on the recurrence of a very rare type of tumour: a highly malignant germ cell yolk sac tumour of the ovary at stage T3bN0M0. The Moscow doctors monitoring and treating the patient disagreed about the strategy for further treatment, because the low incidence of this condition means that no mandatory international or national treatment protocols exist. Having received translations of her medical history extracts and her radiological findings from the patient, we turned to one of our regular consultants in gynaecological oncology, Prof. Dr. med. Nicolai Maass, Head of the Clinic for Gynaecology and Obstetrics at the University Hospital Schleswig-Holstein (Kiel campus). The case was complicated by the fact that the patient was already scheduled for surgery in Moscow and needed the professor's opinion within a very short time so as not to lose her operation date, while at the same time making a considered decision. In just one day Professor Maass assessed Elena's case and drew up a written report, which we quickly translated into Russian for her.
The professor spoke highly of the work done by his Russian colleagues, noting the positive trend in the clinical picture. Given Elena's wish to have children, the professor, himself the father of five children, recommended surgery that preserves fertility. In this case a salpingo-oophorectomy on the affected side is sufficient, combined with inspection of the entire peritoneal cavity, including the pelvic and para-aortic lymph nodes. Removal of the lymph nodes, Professor Maass noted, has no proven benefit provided that they are clinically unremarkable. He strongly recommended lymphadenectomy by removal of the sentinel lymph node using indocyanine green marking as an individual therapeutic concept.
On 29.07.2020 we received the following message from Elena K.: "Alla, good afternoon! Last week I had the operation - everything went well! Today we received the histology results - there are no signs of tumour growth. Tomorrow we are going to see the doctor. Thank you very much from me and from my mother for your understanding and for providing the report so promptly! It helped us to make the right decision."

In February 2019 we arranged an examination and consultation at the ophthalmology department of the University Hospital complex in Aachen for a young woman from Chita, Svetlana R., whose vision in her left eye, operated on in childhood for strabismus, had deteriorated sharply: visual acuity was declining and the image was double. Unfortunately, her visit to the S. N. Fyodorov "Eye Microsurgery" medical centre brought no clarity to the diagnosis and no treatment was prescribed.
